MOA: Bactericidal by damaging bacterial DNA
CI: Known hypersensitivity to the product, anuria, pregnancy (38-42wk), neonates, RI, pulmonary fibrosis, neuritis, polyneuropathy, G6PD
AE: Exanthema, urticaria, pruritus, drug fever, pulmonary HTN, hemolytic anemia, peripheral neuropathy, N/V/D
W&P: Monitor pulmonary & hepatic function during long term Tx & in elderly; anemia, DM, electrolyte imbalance, Vit B deficiency; predisposition to peripheral neuropathy; caution in infants with G6PD deficiency; PRC B, Lact
IND:DOS: Acute UTI: Adults: 200-400mg/ 24h PO div tid-qid; CH: 100mg PO bid; Chronic UTI: Adults: 100-200mg PO qd x 2-3mo; CH: 100-200mg PO qd x 2-3mo; UTI (PRO): Adults: 50-100mg PO qhs; CH>1mo: 1-2mg/kg BW/24h PO div q12-24h, max 100mg/24h; DARF: GFR (ml/min):>60: 100%; <60: not rec
